ACS - Experience Letter - Affidavit Signed by Previous Employer's Colleague

There are cases where the previous employer's work experience cannot be provided as expected by ACS. i.e. either the company is not supportive of generating a new Job experience letter or its closed (as in in my case). Then, you could take available document like Pay slips, relieving letter or experience letter (that might not be in the exact format) and attach that to an affidavit that has been signed by the previous companie's superior (if possible) or some colleague. The colleague would need to sign the affidavit in front of an authorized personelle; a notary in my case.

Following is the template of how the experience letter should look like, for the validation by ACS.

NOTE: I would like to emphasize that this is just a template and it would be great if you could run it through a Notary first before directly adapting the template for your use.
